Providers can apply via the Provider Relief Fund Application and Attestation Portal. The first step is TIN validation which may take several days, so providers are encouraged to begin their application as soon as possible to ensure they are able to meet the deadline. Phase 4 will be like Phase 3, in which HRSA will wait until all applications are filed in order to calculate the percentage of lost revenues and expenses to reimburse.
The application will close on October 26, 2021 at 11:59 p.m. ET. Information on the technical assistance webinars, documents, and information needed to apply can be found in the resources section of the Future Payments HRSA webpage. HRSA will be hosting the Technical Assistance Webinar on four separate dates. We encourage providers to participate before submitting their application.
Providers will apply for both programs in a single application, and HRSA will use existing Medicaid, Children’s Health Insurance Program (CHIP), and Medicare claims data in calculating portions of these payments.
